Your First Visit
Welcome to our practice and thank you for visiting us! We look forward to meeting you and helping you with everything dental!
For your first visit, please bring the following items:

A copy of your dental insurance card (if you have insurance); if the policy holder is someone other than yourself, we will need his or her full name, address, date of birth, and social security number as well

A list of all current medications you are currently take, including prescription drugs, over-the-counter drugs, and vitamins

A note stating you are cleared for dental cleanings and procedures if you have had recent joint-replacement surgery (within the past six months)
If you currently take premedication for dental procedures, please remember to take your premed an hour before your appointment with us. Please see our section on Premedicating with Antibiotics.
If you are transferring from another dental office, you can print out and sign a Transfer of Records form and send it to your previous dental office to have your records emailed to us prior to your first appointment. (The form is found under HIPAA and Other Forms.) If you’d like to fax (336-282-3292) or email the form to us, we’ll be happy to contact your former office to have your records transferred.
We do all patient check-ins and medical history through our waiting-room computer (we are happy to help you with this, if needed). If you wish, you can read a copy of our privacy notice before you come in for your first visit (also found under HIPAA and Other Forms).
